Mud Creek Missionary Baptist Association 
Scholarship Application

Please complete this application and submit it online through our official website by the application deadline. All applications must be submitted in full without exception. The Mud Creek Missionary Baptist Association invites you to apply for its annual scholarship. Recipients will be honored on      September 20, 2025, at the Moderator’s Banquet. The scholarship will be awarded to a deserving nominee from one of the Association’s member churches and will be granted for a period of one year.

Criteria and guidelines for consideration (All must be met to be considered):

 The application must be completed, ensuring all criteria and guidelines listed here are met.

 The applicant must be graduating from high school or be a current college or university student.

   The applicant must have maintained a 2.5 GPA (C+ average) throughout High School or College/ University.

  The applicant must be an active member of one of the member churches.

   The applicant must be enrolled in or accepted to an accredited college or university and pursuing an undergraduate degree. This scholarship is not available to individuals enrolled in graduate or advanced degree programs.

    The applicant must provide proof of enrollment, such as a class schedule or an official letter of acceptance from the institution.

    The applicant must be nominated by their church, which must be in good standing; if the church does not maintain good standing with the association, the applicant is not eligible for the scholarship.

    The applicant (designee) is required to acknowledge receipt of the scholarship by being present at the Moderator’s Banquet to formally accept the award.

     The applicant is eligible to receive the scholarship only once per category—once in the high school category and once in the college.

 Scholarships are awarded based on demonstrated financial need, as well as active involvement in both church and community activities.

The applicant is required to provide a list of all scholarship awards received or submit a statement confirming that no scholarships have been granted.

Please provide details of any additional school, church, or community activities or awards that highlight your qualifications and distinguish you from other candidates.

Applicants are required to submit three letters of recommendation, one of which must be from the applicant’s pastor. If the church is without a pastor at the time of submission, a letter of recommendation must be provided by the Chair of the Deacons.

  The applicant must submit a 500-word essay on “Why I Deserve This Scholarship.”

I authorize the release and disclosure of all relevant academic and personal information to the Mud Creek Missionary Baptist Association Scholarship Nomination Committee. This includes, but is not limited to, transcripts, academic records, personal evaluations, and any other documentation maintained by secondary school and/or post-secondary institutions or other relevant parties. I further acknowledge and consent to the potential publication or public disclosure of such information in connection with the announcement and awarding of the scholarship.
